Cognitive Care Manager – Cognitive Care Management
Job Description
The Cognitive Care Manager role is an exciting new role for those who are creative and have a passion for working with individuals with cognitive conditions.
This person will be responsible for a portfolio of Cognitive Care Management clients, working as a consultant and coach to help clients and their families cope with the effects of cognitive conditions. This person’s objective will be to ease the burden of navigating life decisions that accompany cognitive conditions so our clients can focus on participating in life more fully and independently. The person will develop a strategy roadmap, curate a personalized toolbox of digital content and activities, support disease education and drive collaboration between the professionals and caregivers in the care of our clients.
Type: We are offering a part-time position.
Location: Our services are delivered virtually, therefore this position is open to those living anywhere in the continental United States.
Responsibilities and Essential Duties
The position requires a strong technical understanding of a range of cognitive conditions and excellent interpersonal and communication skills to manage the day to day needs of our Concierge Membership clients. The position also requires a curious and creative mind to develop and deliver personalized advice and content for our clients to ease the burden of navigating life decisions.
- Direct client interaction through scheduled virtual meetings (50%)
- Development of personalized Resource Page with strategy roadmap and toolbox for each client (20%)
- Drive collaboration between the professionals and care givers in the care of our clients (20%)
- Conduct research and build out knowledge base to support the requests of our Concierge Membership clients (10%)
Qualifications (Education, Experience and Skills)
- Master’s Degree or PhD/SLPD from an accredited Speech-Language Pathology (SLP) program
- 5+ years of experience working with individuals coping with neurodegenerative conditions (i.e., Alzheimer’s, primary progressive aphasia) and/or neurogenic conditions (i.e., stroke, traumatic brain injury)
- Relationship management and customer service oriented mindset
- Excellent verbal, written and interpersonal communication skills
- Tech-savvy with the ability to learn quickly
- WordPress experience a plus

Cognitive Care Manager – Cognitive University
Job Description
The Cognitive Care manager role is an exciting new role for those who are creative and have a passion for working with individuals with cognitive conditions.
This person will be responsible for the development and execution of our Cognitive University, which are month-long (4 week) virtual events that provide our clients with an opportunity to connect with experts and network with others who have similar cognitive conditions. This person’s objective will be to enhance quality of life by providing new tools and strategies for clients with cognitive conditions. The person will also be responsible for conducting primary and secondary research to support the support the day-to-day needs of our Concierge Care Management clients.
Each month-long (4 week) class showcases a destination (i.e., Boston) or topic (i.e., Ornithology) through curated summaries of history, culture, nature and food. New and different aspects of the destination or topic are brought to life each week through text, images and video on our class webpage. Each week has 4 to 5 hours of content organized into 5 categories of engagement: a guided video from one of our Cognitive Care Managers, a guided online excursion or immersion, multiple cognitive enrichment exercises, supplemental cognitive activities and a weekly video conference social group. Enrichment activities may include articles, videos, cognitive/language exercises, strategy training and culinary, music and art engagement. Participants complete virtual and hands-on activities independently and meet once a week for 90 minutes with their fellow group members and Cognitive Care Manager for a virtual social group.

- Develop new Cognitive University virtual content (30%)
- Coordinate logistics and communication with clients prior to Cognitive University (20%)
- Lead virtual social groups (broken out by various cognitive conditions) of Cognitive University clients (40%)
- Conduct research and build out knowledge base to support the requests of our Concierge Care Management clients (10%)
Qualifications (Education, Experience and Skills)
- Master’s Degree or PhD/SLPD from an accredited Speech-Language Pathology (SLP) program
- 2+ years of experience working with individuals coping with neurodegenerative conditions (i.e., Alzheimer’s, primary progressive aphasia) and/or neurogenic conditions (i.e., stroke, traumatic brain injury)
- Prior experience leading virtual support groups or delivering telehealth services a plus
- Experience developing and producing web or digital creative content
- Excellent verbal, written and interpersonal communication skills
- Tech-savvy with the ability to learn quickly
- WordPress experience a plus
